Field Service Report
The Field Service Report (FSR) was designed to provide vital information from the field including product movement, condition of equipment, parts usage as well as billing information for the technical service provider. A stock of these forms should always be kept with the Technical Service Manual. An FSR should be completed for repair of equipment, installation of equipment or preventative maintenance. Accurate information on the FSR will also help us determine if the equipment is being maintained properly. It is essential that this document be filled out completely and properly. A sample form is provided on the following page.
Instructions for use are as follows:
- Technical Service Representative will be notified of service needed via telephone.
- Technical Service Representative will record Workorder number, customer information, service company information and reported problem on FSR when dispatched.
- During the course of the service visit each section of the FSR should be completed and noted accordingly.
- Have FSR signed or stamped by customer. Leave blue Store Copy with customer.
- Fax white Original to Teazzers at 214-956-0374 to complete service call.
- Mail white Original and yellow Billing Copy to Teazzers with your invoice.
The 4 part carbon-less form contains:
- Original
- Yellow Billing Copy
- Pink Service Provider Copy
- Blue Store Copy
